RETURNING ATHLETES

  • Athletes may begin the online Presagia documents after June 1. You will need your insurance card and immunization record. 
  • Returning athletes will not need a new physical or Health History Form but will need to update all other information online in their Presagia account. https://sports3.presagiasports.com/iZoneWeb/  
  • Reminder: When logging in to your account, be sure you click on the “Notebook or PC sign-in” button and NOT the “Mobile sign-in” button. 
  • All athletes will need to complete this process to be cleared to participate in any varsity sports on campus. 
  • The documents will be reviewed by the Athletic Training Staff to approve clearance for participation. 
  • You will be given an athlete number that you use to “Sign In” at the kiosk along with your last name. Your athlete number will appear at the very top of your profile on Presagia, to the right of the slot for an account photo.
