New Athletes

 

 

 

NEW ATHLETES

  • Athletes may begin the online Presagia documents after June 1.  You will need your insurance card and immunization record.
  • All new athletes will need to get a physical done by an MD or DO on the SDCC Physical Examination Form. Click on the hyperlink to get a printable PDF of the form. (create a hyperlink for the SDCC Physical Examination Form to the PDF)
  • All new athletes will also need to complete the SDCC Health History Form in its entirety. Failure to bring the SDCC Physical Exam and Health History Form upon athlete clearance day will result in a delay of participation until these forms are completed.

*We will not accept forms from CVS, Kaiser, AFC Urgent Care, or any other entity's physical examination form. (create a hyperlink for the SDCC Health History Form to the PDF)

Presagia Account Sign-Up (required)

  1. Click on the link   
  2. Once on the website, click on "Athlete Sign-Up"
  3. Once in the "Athlete Sign-Up", fill out all the empty information fields you see available.
  4. You may use any email address, just be sure it is an email address you use regularly.
  5. Organization Code: sdcchawks (no caps, no space)
  6. Type in the activation code that was sent to the email you used for your profile registration Hint: If you do not see it in your inbox, then check your junk or spam folder for the email with your activation code.
  7. Set your password
  8. Add your phone number
  9. Set communication preference to “Primary Email”
  10. Once you click “Next” you will be taken to your profile where you will have access to complete your required athlete participation documents.

*NOTE:

  • These must be filled out in their entirety to be cleared for sports participation.
  • All athletes will need to complete this process to be cleared to participate in any varsity sports at SDCC.
  • The documents will be reviewed by the Athletic Training Staff to approve clearance for participation.
  • When logging in to your account, be sure you click on the “Notebook or PC sign-in” button and NOT the “Mobile sign-in”
  • When you come to receive treatment or get an injury or evaluation or anytime you need help from us, you will be required to sign in to our Front Desk Kiosk at the entrance of the Athletic Training Room.
  • You will be given an athlete number that you use to “Sign In” at the kiosk along with your last name. Your athlete number will appear at the very top of your profile on Presagia, to the right of the slot for an account photo.
